CCPA request form (California residents only)

If you are a resident of California wishing to exercise your rights under the California Consumer Privacy Act (CCPA), other than your right to opt out of the sale or sharing of your Personal Information, you may use the form below to submit your request or you may refer to ‘How to Submit a Request’ in our California Consumer Privacy Act Disclosure & Statement of Rights for additional contact options.  You also may authorize another person or a company to submit a request on your behalf. For example, you may use this form to:

  • Request that we delete Personal Information we have about you
  • Request that we correct errors in Personal Information we have about you
  • Request information about the categories of Personal Information we have about you or to request copies of specific pieces of Personal Information we have about you
  • Request information about the categories of Personal Information that we have about you that we have sold to or shared
  • Request information about the categories of Personal Information we have about you that we have disclosed to others for a business purpose
  • Request information about categories of persons to whom we have sold or shared Personal Information we have about you or to whom we have disclosed Personal Information we have about you for a business purpose

In completing your request, please provide only the information necessary to answer the questions set forth below.

 

Once you submit your request, we will promptly review it to determine its authenticity. It may be necessary to obtain additional information from you in order to validate and process the request. For example, we may need additional information to confirm the identity of the individual that the request pertains to (and of the requester if different than the individual that the request pertains to) and to clarify your relationship with us (and your relationship with the requester if someone else is submitting this request on your behalf).

Once we have determined that the request is valid and have obtained the information we need to process the request, we will process the request within the time period required by applicable law and provide the results to you using your preferred method of contact.
